Heavy amounts of violence and especially gore persist throughout the film. Multiple characters are shown missing limbs, having body parts blown off of them violently, and being shot and stabbed.
This is probably the goriest TF2 movie. Its ranged from mild blood to extremely graphical dismemberment. Most of the death scenes are heavily unsettling and shocking. Gore is present in almost the entire movie.
A man is struck in the leg with an axe, which is then plunged into his neck off-screen (blood spray is seen) before the camera shows his decapitated body. Later his severed is used to trick a character before the axe is stabbed into his chest and he is shot in the head (a wall obscures the death but his blown apart head is seen).
A man is tortured to death with a drill. The death isn't seen but the sounds are heard and are very disturbing.
A woman's decapitated head is briefly seen along with a blurred-out shot of her body. Although not graphic, this is extremely disturbing and sudden.
A man strangles another man and smashes his head against a trash can but before he can kill him, he is shot in the chest by another man with a pistol. Mild blood is seen.
A man is shot repeatedly with a machine gun, causing his torso to be torn apart and blood to splatter everywhere. He begins to laugh and is killed by being shot with a shotgun although this is off-camera, with a brief shot being shown of blood seeping from his destroyed head.
A man falls and hits his head on countertop, with this implying to have killed him.
A man shoves a crowbar into a zombie's head and drives it in further. This is extremely graphic with blood splattering from the wound.
A man's eye is gouged out with a screwdriver, blood is shown spraying everywhere. Non-fatal but bloody.
A large man grasps the head of another in his hand and squeezes, crushing it. This is very brief and not graphic as it is seen from a distance although some blood is shown seeping through the fingers.
A man is shot in the head and begins to convulse from the wound in his skull before turning into a zombie, very surreal.

Although the violence is not as exaggerated as other TF2 movies, it is undoubtedly the most realistic.
In two separate instances throughout the film, a man is shown with empty, frayed eye sockets.
Emesis blue has the highest kill count of any film ever made. With this it is guaranteed VERY violent.
A man's head is repeatedly smashed against a piano. The scene is gory.
